With 3 new Saudi MERS cases, study notes antibodies in Pakistan camel workers

November 18, 2020

first_imgSaudi Arabia’s Ministry of Health (MOH) today reported three new MERS-CoV cases, part of an ongoing increase that began in October, and new research reports reveal more information about the presence of antibodies to the virus beyond Saudi Arabia in both people and camels. The new MERS-CoV (Middle East respiratory syndrome coronavirus) illnesses lift Saudi Arabia’s number of cases for November to 5. The brisk pace of infections so far this month follows 14 reported for October, which was up sharply from 4 in September.Camel exposure in latest Saudi casesThe three newly confirmed patients include a 64-year-old man from Riyadh, a 73-year-old man from Wadi ad-Dawasir, and an 83-year-old man from Buraidah. Riyadh and Buraydah are in central Saudi Arabia; Wadi ad-Dawasir is in the south central part of the country. Other recent cases have also been reported in the three locations, including one announced on Nov 3 involving a woman from Wadi ad-Dawasir who has a secondary infection. All three of the latest confirmed patients had contact with camels, the most common source of the virus. They are also listed as having primary exposure, meaning they probably didn’t contract MERS-CoV from another known patient. The World Health Organization said in a recent update that since the virus was first detected in humans in 2012, it has received reports of 2,470 cases, at least 851 of them fatal. The vast majority are from Saudi Arabia.Antibodies in Pakistani camel handlersPakistan hasn’t reported any human MERS-CoV infections, but an international research team conducted a blood sampling study to look for antibodies to the virus in camel handlers in the Cholistan desert in Punjab, the country’s most important camel industry area. They published their findings today in Emerging Infectious Diseases. In 2017 and 2018 the team obtained blood samples from 100 camel handlers and their families from a range of the area’s nomadic, seminomadic, and sedentary populations. They also collected clinical data and information about exposure to camels and camel products, such as milk. They tested for MERS-CoV antibodies by ELISA and 50% plaque-reduction neutralization testing (PRNT). Of 91 samples tested by ELISA, 49 were positive for MERS-CoV antibodies. Twelve were positive by PRNT, and, of those, 5 were also positive by ELISA. Immunofluorescence testing was positive for 10 of the 12 PRNT-positive samples.Of the 12 patients who were positive on PRNT, 8 were men, 3 were women, and 1 was an 8-year-old child. All but two of the participants were exposed to camels. Though the sample size was small, the researchers found no significant correlation between MERS-CoV seropositivity and lifestyle, underlying medical conditions, drinking unpasteurized camel milk, or tobacco use.Though the mechanism for MERS-CoV transmission isn’t clear, the group said drinking fresh milk may be a source of infection and shared water sources between camels and humans in the area of Pakistan may also play a role. They concluded that more studies are needed to explore clinically apparent infections in Pakistan’s camel regions and that it’s important to educate camel handlers about infection control measures to reduce the risk of MERS-CoV transmission.Looking for MERS-CoV evidence in SudanIn a separate study in Emerging Infectious Diseases today, a separate group of researchers detailed their results of blood testing for MERS-CoV antibodies in Sudan camels, camel workers, livestock, and other animals. They collected samples from humans and animals in 2015 and 2017 in Tamboul, Sudan. Sites include a live animal market and an adjacent slaughter area. They also collected samples from 90 Sudan camels imported into Qatar in 2015.In Sudan, tests were done on samples collected from 56 humans, 190 camels, 3 bats, 14 donkeys, 15 cows, 15 sheep, and 15 goats. In 2015, 92% of camels in Sudan and 99% of camels exported to Qatar were positive for MERS-CoV antibodies. In 2017, all camels tested in Sudan were seropositive, but tests were negative for the people and other animals. Also, MERS-CoV RNA was detected in nasal swabs from 3 camels imported into Qatar. The team concluded that the results of the study are consistent with other serologic studies of MERS-CoV in Africa, but the results aren’t conclusive, given the low numbers of human and livestock samples tested.”However, this study provides preliminary insight into MERS-CoV circulation in Sudan, the country with the third largest dromedary camel population in the world,” they wrote.See also:Saudi MOH epi week 45 reportNov 6 Emerg Infect Dis Pakistan studyNov 6 Emerg Infect Dis Sudan, Qatar studylast_img read more

Cyril Glasser remembered

first_img Diane Long , executive assistant, Joint Committee for Ethnic Minorities in Wales The Gazette’s report of the death of Professor Cyril Glasser did not allude to the fact that he was an outstanding and much-loved law teacher. Some of our members attended his seminars at University College London, where he delivered weekly lectures on civil procedure with specific reference to the Woolf proposals. Our members still remember with gratitude and much affection the fact that his seminars were linked to a few leading cases, the analysis of which supplied the rationale behind the various orders set out in the new Civil Procedure Rules. He would invite law lords and lords justices to address his class to defend or explain decisions with which the academic lawyers disagreed. And because of the large presence of American lawyers and scholars at these seminars, he would regularly remind participants of the words of Abraham Lincoln (a notable trial lawyer) in civil proceedings: ‘Discourage litigation. Persuade your neighbours to compromise wherever you can. Point out to them how the nominal winner is often a real loser in fees, expenses and waste of time. As a peacemaker, the lawyer has a superior opportunity of being a good man. There will still be business enough.’ Professor Glasser was a good man. Our condolences go to his family.last_img read more

Something in the air

September 29, 2020
| zntjinwn

first_imgIt took just five days this year for London’s air pollution limit to be breached, on Brixton Road in Lambeth, south London. A fortnight later, air quality in the capital was worse than in smog-filled Beijing.Air pollution is estimated to contribute to around 40,000 early deaths in the UK each year. Globally, according to the US Institute of Health Metrics and Evaluation, it contributes to more premature deaths than both high cholesterol and alcohol. Yet aside from the occasional mask-wearing cyclist, the problem has, so far, appeared to have made little impact on British public or political consciousness. That now seems set to change. London mayor Sadiq Khan has made tackling air pollution his “number one priority” – above policing, improving transport links and addressing London’s housing crisis. And the construction industry – full of site-based plant that so far has not had to meet the emissions standards of road-based vehicles – is firmly in his sights. There has been a growing amount of research into pollution from construction sites that offers evidence of its impact. The London Atmospheric Emissions Inventory estimates that 12% of London’s damaging nitrogen oxide emissions come from site-based plant and machinery, along with 15% of emissions of coarse particulate matter, known as PM10.Initiatives have been introduced to help combat the problem; most notably, London’s 2014 planning guidance and Ultra Low Emission Zone, a pair of initiatives that set minimum performance standards for site plant and machinery. But with estimates of the scale of the problem rising, and existing standards in the capital reliant on London boroughs first choosing to implement the standards and then finding the money to police them, existing measures appear increasingly inadequate. At least, they do to Khan.He is now lobbying the government for the power to set up a kind of congestion charge for site plant, which developers can avoid only by using clean, up-to-date equipment.12% of London’s damaging nitrogen oxide emissions come from site-based plant and machineryWith industry output forecast to flatline amid uncertainty around Brexit, and construction firms facing a squeeze on margins thanks to rising labour and materials costs, construction bosses may well think that another charge is the last thing they need right now. Particularly since the public sector is pushing for faster, cheaper delivery on everything from homes to railways. But with around 9,500 deaths in the capital each year caused by poor air quality, and a single modern excavator – one which meets regulatory standards – estimated to churn out 15 times the emissions of a modern double-decker bus, there is a clear moral imperative for construction to up its game. And not least because of the impact on its own workers. The silent mounting toll of spending years breathing in diesel fumes from plant has already been compared to the previously hidden harmful effects of asbestos by Daniel Marsh, an air quality analyst at King’s College London. This week, Marsh warned that companies were “essentially poisoning their own staff” (see news analysis). For businesses that still think “yes, but why now?” when it comes to upgrading to cleaner plant, Khan’s stance offers a clear signal over the direction of travel in London. Should he fail to secure powers to charge firms, it’s a sure bet that he will look to toughen regulations through any other means available.Meanwhile, as other UK cities – particularly those with greater devolved powers – implement low emissions zones, it is likely they will follow the capital’s lead. As such, failing to invest in cleaner equipment, particularly as machinery comes up for replacement, looks a risky tactic. In the longer term, having a modern construction industry that makes the most of technological advances for efficient, safe working, is a vision few in the sector would argue with. If Khan is successful in bringing in his charging system, construction firms may well want to debate the phasing in of plant requirements, and the level of fines involved. Perhaps the sector would have a case to push for tax incentives for investment in the cleanest plant. But it should line up behind the principle of the plans.Because construction should be, and be seen to be, an industry whose work improves people’s lives. Not one that puts them at risk.Sarah Richardson, editorlast_img read more
